- If you received an email notifying you that you have preloaded or gift card credit on your account that can be refunded after 9/30, please send us your info through this form. There, you will be able to choose if you would like a refund to your card on file within the LevelUp app or if you would like to receive a check.
- If you have gift card or preloaded credit specifically from one of the following restaurants, we are unable to provide a refund:
- Fresh to Order, Kwench Juice Cafe, Mighty Quinn's BBQ, Spring Chicken, The Juice Box, Vino Fine Wine & Spirits.
